FDA Clears Gilead’s Once-Daily HIV Tablet for Complex Treatment Needs

The new drug combines bictegravir—the core component of Gilead’s widely used Biktarvy—with lenacapavir, a first-in-class capsid inhibitor. While many people living with HIV currently maintain viral suppression through a single-tablet regimen, approximately 5% of U.S. patients require more complex combinations due to side effects, drug interactions, or historical resistance to older therapies. For these individuals, Bixlenvo serves as a critical bridge to a more manageable daily routine.

Gilead has set the list price for a 30-day supply at $4,595, positioning it alongside existing single-tablet treatments. The company also confirmed that patient assistance programs will be available to mitigate costs for those without insurance or those requiring co-pay support. Dr. Jared Baeten, head of clinical development and virology, noted that the pill is particularly significant for aging patients who have weathered decades of treatment history and now manage the accelerated health complications associated with the virus.

Beyond stabilizing complex cases, Bixlenvo expands the portfolio of options for patients seeking alternatives to their current therapy. The release aligns with Gilead’s broader strategy to diversify HIV care, which includes developing weekly oral regimens and long-acting injectables. By offering a range of delivery methods, the company aims to move closer to a future where patients can tailor their treatment to their specific lifestyle needs, whether that involves a daily pill or long-term injectable options.
